Items Needed:

  • Federal identification number.
  • State withholding number and coupon book.
  • City withholding number and coupon book, if applicable.
  • State unemployment number and experience rating.
  • A copy of one of your checks and a starting check number.
  • Names, addresses, social security numbers, and filing status of employees. (Forms W-4)
  • Pay rates and/or salaries.
  • Copies of all year-to-date Forms 941 and State Wage and Contribution Returns.
  • Employee wage summary reports, year to date.
  • Records of federal, state and city (if applicable) deposits, year to date.
